But… the doctor is a member of a profession with ethical obligations. Demand doesn’t change that. The ethical mad-scientist-doctor would experiment on him/her self, as is longstanding tradition.

people like the guy in the email will never take “no” for answer so I think my middle ground answer is that you exclude yourself from certain types of care and have to cash pay if anything goes wrong Why should his fellow policy holders have to pay for the outcomes of this choice of his
