Yes, we are. Ideally, we should have a higher vo2 max because turning to breathe often slows us down.

How do you improve it? Do you run or cycle just for improving vo2 max?

If stair stepper brings you to zone 2 and HIIT is somewhat structured - e.g. Norwegian 4x4, Tabata style intervals - than it looks like you are doing the right thing already. I was more surprised there is no specific swimming workout for improving vo2 max (I know almost nothing about swimming)

I donโt think that there are swimming workouts for improving vo2 max because the water naturally cools down and itโs hard to get to Zone 2? Not sure as I am not well educated on vo2 max.
