Ceres-1 exploded for a 2nd time over the past 4 years, with 20 successful missions. The two explosions are 2 year apart, the first in 2023 and the second in this late 2025. That is 9% failure rate. Is that high in engineering in general, and in rocker launching business?

How does the practice of "chaos engineering" look like in non-software projects?
I wonder if there are typical chaos-like tests done across engineering disciplines? Like "what happens if I remove this screw, or cut this rope", etc.

One of the reasons I love aerospace is that you can’t fake building an aircraft. You can’t growth-hack orbital mechanics. You can’t make safety critical systems without really thinking things through and engineering them
