having trouble adding a user to a group chat (size of group is 3). i go through the flow but nothing happens once i click "add members".

Depends on the user settings but yes — very likely they may not be following you.

how do i get perms to message? we have to follow ea other?

We need to do a better job on this on the UX but what is likely going on is that one of the members you are trying to add a member you don't have permissions to message.
