I used to think that was my issue because I have shoulder defects, but I found that it was my form. Some things that help me - suicide grip, palms out, helps me engage my back, especially near the top of the movement. - using my abs to push my toes forward helps keep my weight back, giving more mechanical advantage - from dead hang, take the slack out of the bar by pushing the torso up fully with your back before starting to close the armpit or elbow. Some people say "put your shoulder blades in your back pocket" - using the queue "pull the bar down to my nipples" helps me get into position behind the bar, not under it. I also hear people say "Pull the elbows down to the floor" too.
