stress, probably. the second rule of Hippocratic Oath is to keep yourself healthy so that you can be the most effective healer. but in countries like the United States, there's a tightly-held culture to sleep-deprive and underadmit med students. there's a narrative violation when they can consult at a distance, even across borders.
