OrthoIQ is work in progress and here are the latest updates. I’ve posted some recently about limitations in AI healthcare. One voice. No accountability. No outcomes tracking. I’ve been slowly working on building an answer, hopefully. ➡️ OrthoIQ. I’ve noticed as a practicing orthopedic surgeon, the ever-increasing utilization of ChatGPT and other LLMs for healthcare information and diagnosis. I’ve used these models myself and while I’ve been extremely impressed overall, there are shortcomings. Given the nuances of healthcare, I wanted a doctor in the loop rather than an AI model speaking definitely. Medicine, and healthcare, is not always a cookbook and opinions still matter. So, instead of one LLM voice giving one opinion… OrthoIQ now uses a panel. Five specialized agents — each with a different lens on your case. A triage master coordinator, a pain specialist, a movement analyst, a rehabilitation specialist, a psychological recovery expert. All of them reviewing your case. All of them producing independent assessments. All of them accountable for their recommendations. That’s OrthoIQ. Here’s the part that has no parallel in consumer health AI right now: For clinical-grade questions, each agent stakes tokens on predicted outcomes. If their recommendation leads to measurable improvement (tracked via validated clinical instruments at 2, 4, and 8 weeks), they earn. If not, they lose. This is a prediction market applied to medicine. Agents that are accurate accumulate. Agents that are wrong get penalized. Over time, the system gets smarter. Not by being retrained. By being accountable. You get an immediate triage response in ~25 seconds. Then, if you want to go deeper, the specialist panel fires. Pain Whisperer, Movement Detective, Strength Sage, Mind Mender all produce independent assessments. A Research Agent pulls live PubMed citations in real time. The result is a structured brief that reads like a multidisciplinary consult note. With citations. ✅ Every flagged consultation hits my review queue. MD oversight. ✅ The MD oversight isn’t a disclaimer, it’s an architecture decision. We’re heading into testnet in the next couple of weeks. The token mechanics go live.
