Anyone building agnostic agents for the patient? 

Seems like in the pursuit of distribution, AI startups are going B2B, portability will be just as much, if not bigger problem. 

As health data becomes a strategic asset, incentives don’t line up to share info. And patients have seemingly less control.

Yup along those lines. Essentially something that has the patient's interest in mind. 
A health systems agent can be trained to refer the patient internally and avoid out of network referrals even if that is beneficial for the patient.

Like agents controlled by the users or provider agnostic so new providers can come up to speed without friction?

For the former I’d say that’s the direction function health is headed
