I’ll list out where I missed the mark, conduct the 5 whys on each, and generate action items. I like this structure because it focuses on the learning.

As with most things, it depends on the context. But in broad strokes, culture. You can roughly increase inclusivity if your culture supports high standards and expectations. Those who rise to the occasion swim, those who don’t, sink. Give “cultural runway” a look.
